An event to continue work on developing plans for providing health and social care for more people at home or closer to home will be held on 8 May.
Anyone interested in giving their views on the subject is welcome to take part.
The meeting about Care at Home/Closer to Home – joining up services is a follow-up to one held on 30 January this year where people gave their views on current services and highlighted things to consider when planning for the future.
Information from the event – along with information from other activities to find out people’s views – has been put into a report produced by NHS Greater Huddersfield CCG. You can find more information and a link to the report here.
NHS North Kirklees CCG is continuing to talk to key stakeholders over the next few weeks to help shape and develop their proposals. Those activities will be included in their report, which will be sent out with the agenda for next month’s meeting at a later date.
The meeting on 8 May will involve participants working together to help shape the integrated service for Care at Home/Closer to Home. Those taking part will look at what was said previously, how this has been reflected in the proposals and discuss what else is needed.
The event will be held on Thursday 8 May between 9.30am and 12pm at the Hudawi Cultural Centre, Great Northern Street, Huddersfield, HD1 6BG.
If you would like to attend the event please complete this booking form and send it to Zubair.firstname.lastname@example.org by Friday 2 May.
